Medi-Weightloss Clinics® is excited to announce an upcoming promotion- The Recipe Challenge! This contest allows our patients to submit their favorite recipe and have the chance to win prizes! Please review the official rules and regulations (below).

The Medi-Living Recipe Contest judges will review entries & select 10 semifinalists based on the criteria set forth in the Recipe Contest Official Rules (see official rules below). Those 10 semifinalists will be featured on the Medi-Weightloss Clinics® Facebook page. www.facebook.com/1.877.MED.LOSS Visitors will be encouraged to vote for their favorite semifinalist. The semifinalist with the most votes will win the grand prize.

1. Contest is open to legal residents of the 50 United States and Washington, D.C., who are either former or current patients of Medi-Weightloss Clinics®. Contest not open to current employees (and their families) of Medi-Weightloss Clinics®. Void where prohibited by law. Contest begins 9:00 A.M. EST April 1, 2010 and ends May 31, 2010 at 11:59 P.M. EST. One entry per person.
2. Create and submit one original Medi-Weightloss Clinics® program approved recipe on the Facebook page at http://www.facebook.com/1.877.MED.LOSS?ref=ts Entrant represents and warrants that the submitted recipe is their own original recipe and does not infringe any privacy, intellectual property or other rights of any third parties. By entering, entrant represents that he or she owns or has the rights to convey any and all right and title in the submitted recipe and photo; and by entering, entrant conveys and assigns to Medi-Weightloss Clinics® all rights of ownership, title, interest and all other intellectual property rights, including without limitation, copyright in and to the recipe and photo. All entrants, by entering the contest, grant Medi-Weightloss Clinics® a non-exclusive, worldwide, royalty-free copyright license to publish their submitted photos and recipes, and to republish them at any time in the future in any and all media.
3. All entries must be via the Medi-Weightloss Clinics® Facebook page. No entries will be accepted via any other method.
4. Only photos submitted following the directions and in electronic formats .jpg or .gif, with images at least 72 dpi and 3 Mbs or smaller will be considered as online entries. Online entries must be received before 11:59 P.M. EST on April 30th, 2010.
5. Ten semi-finalists will be chosen by a panel of judges selected at the sole discretion of Medi-Weightloss Clinics®, based equally on recipe and photo. Judges' decisions are final and may not be appealed. The ten semi-finalists will be posted on the Medi-Weightloss Clinics® Facebook page by May 15, 2010. Semi-finalists must sign an affidavit of eligibility; a liability release; where legally permissible a publicity release; and confirmation of a license as set forth above in the form requested by Medi-Weightloss Clinics®, and return them within seven days of notification attempt, or alternates may be selected. The ten semi-finalists will be featured on the Medi-Weightloss Clinics® facebook page from May 15-31, 2010 and the winners will be based on viewer’s votes. Only one vote will be accepted per registered e-mail address. The top-scoring finalist will be declared the "Grand Prize Winner" of the Recipe Contest and their recipe will appear in Medi-Weightloss Clinics® Medi-Living. The winner must submit a release form allowing Medi-Weightloss Clinics® to utilize their recipe and photo. Individuals not willing to fully comply with all instructions, rules and regulations associated with this prize will be subject to removal and another winner will be chosen. Each Contestant by entering the Contest agrees, represents and warrants that he/she will be bound by the Rules and the Sponsor's decisions and Privacy Policy. Disqualification and, time permitting, the selection of an alternate semi-finalist, may result from any non-compliance with the rules.
6. The winner will receive $250, 1st runner up will receive $100, 2nd runner up will receive $50, and places 3-10 will receive a Medi-Weightloss Clinics® gift bag valued at no more than $50.
7. The top three recipes will appear in an issue of Medi-Living. The top three recipes will also be featured on the Medi-Weightloss Clinics® Facebook page. If the winner is unreachable, or if prize notification is undeliverable, or in the event of non-compliance with any of these requirements, the prize will be deemed forfeited, and an alternate winner may be selected.
8. In the event of any tie, representatives from Medi-Weightloss Clinics® will determine the winner based on their own judgment and in their sole discretion.
9. Retail value of prizes is as follows: Grand Prize Winner -- $250, First Runner-up -- $100, Second Runner-up -- $50.

All taxes are the sole responsibility of semi-finalists', finalist's or Winner's. Prizes are non-transferable and cannot be substituted, but prizes of equal or greater value may be supplied by Sponsors at Sponsors' sole discretion.

10. All entries become property of Medi-Weightloss Clinics® and will not be acknowledged or returned.
11. Medi-Weightloss Clinics® is not responsible for technical failures in entry transmission, or lost, late, misdirected, damaged, incomplete, illegible, or postage-due mail, or any damage to participants' computers relating to information about or participation in the promotion. Medi-Weightloss Clinics® reserves the right to cancel electronic method of entry if it becomes technically corrupted.
12. By entering the contest, entrant holds harmless Medi-Weightloss Clinics® and their respective parent, subsidiary, and affiliate companies or operating divisions, from any liability whatsoever for any claims, costs, injuries, losses, liability, actions or damages of any kind or in connection with: (1) entering contest (2) acceptance of any prize or (3) anything related to the contest and the awarding of prizes. Any claims relating to this contest shall be governed by Florida State Law without regard to its conflict of law rules, and any such claim must be heard in Florida.
13. If any provision of these Rules is found to be invalid or unenforceable by a court of competent jurisdiction or appointed arbitrator, such determination shall in no way affect the validity or enforceability of any other provision herein.
